Return Materials Authorization (RMA) - Request Form

Instructions:

1. Fill in this form as completely as you can. All fields marked with the red asterisk are required and the form will not submit unless data is provided.
2. After submission the results will be emailed to our returns department and will be processed in the order they are received. Once we have processed your request we will contact you with an RMA number and a copy of the data submitted through this form. You MUST include both the RMA number and the copy of the form data with your return for it to be accepted.
3. There is a $60 charge for product that is returned and no problem can be found with it or is determined not repairable. Non-warranty repair work is separate from this charge.
